The Catharina Ziekenhuis runs a dedicated IBD-transitiespreekuur, a transition clinic that prepares adolescents (roughly 16-18 years) with Crohn's disease or ulcerative colitis for the move from paediatric to adult care. The multidisciplinary team includes a kinderarts-MDL, an MDL-arts and an IBD nursing specialist, and an IBD nurse specialist advice line is available daily.
